  • The referral from your doctor
  • Your x-rays, ultrasounds and pathology results.
  • A list of other illnesses and/or medications.
  • Any specific questions you would like answered.
  • Your Medicare, DVA or Pension card as applicable.
  • Your Health Fund details.
